These interviews have not been published in a book as such. The title and chapter numbers are used on the CD-ROM, and also for the naming of audio and video media.
Chapter 11 (date unknown) is missing on the CD-ROM, it may be an interview that was not recorded, or missing. See the discussion for more on this.
Chapter 15 (86-08-14-5) does exist (there is audio and video), but has apparently not been transcribed and did not make it onto the CD-ROM.
